RA是指冠状动脉内血栓形成后自溶或药物溶栓、PTCA等方法使闭塞的冠状动脉再通及冠状动脉痉挛的缓解等恢复心肌再灌注所致的心率失常。随着静脉溶栓的普及和急诊PTCA的开展,再灌注性治疗所致的心率失常越来越引起我们的重视,本文对RA的临床情况进行分析。从资料中显示再灌注治疗患者出现心律失常75人次,占再灌注人数52?4%。非再灌注治疗患者出现心律失常17人次,占非再灌注人数26?2%。由此可见AMI患者出现心律失常是以再灌注后多见,而且RA是以室性心律失常为主。AMI后RA与缺血性室性心律失常发生的机理不同。动物实验表明,冠状动脉梗阻后缺血区域动作电位时间、0相除极速率降低、不应期增加、传导减慢、室颤阈值下降。由于连续的舒张期损伤电流的存在,在边缘区容易引起缺血性心律失常。相反,由于突然再灌注,室阈值降低伴有明显的不应期缩短,并增加了动作电位的离散,舒张期电位在许多病例不明显。RA在AMI在灌注者中发生率较高,文献统计发生率为20%~80%左右[3]。而本文RA的发生率是45%,在文献统计的范畴内。引起RA的机理??[1]有:①毒性氧自由基对心肌的作用:氧自由基在正常情况下有一小部分氧经单价途径还原为水,其中间产物有超氧自由基、过氧化氢和羟自由基,它们对组织细胞具有毒性作用。②局部心肌电生理异常:再灌注后缺血区域心肌恢复供养,可使存活的心肌加速走向坏死或心肌仍处于非同步的病理状态,易于形成折返性心率失常。③局部心肌代谢异常:心肌缺血以后,细胞内ATP合成减少丧失了排钠转钾能力。恢复再灌注、超负荷的水钠、钙离子大量进入细胞内,细胞内水肿,钙盐沉积,导致细胞水肿、肌原纤维断裂,线粒体肿胀,钙离子超负荷可经一系列途径导致心细胞膜损伤发生RA。④室颤阈值降低:再灌注心肌室颤阈值降低也是RA的一个重要因素。⑤后电位:早期后除极及延迟除极也是产生RA的重要因素。⑥室性异位起搏点自律性增高:室性异位起搏点自律性轻度至中度增高分别产生加速的室性逸搏心律及阵发性自律性室性心动过速。因此AMI再灌注治疗前及治疗过程中,应用抗心律失常药物,预防RA 的发生。AMI由于冠状动脉急性闭塞也可以引起心律失常,特别是在冠状动脉闭塞早期(冠状动脉闭塞即刻至30 min),引起冠状动脉闭塞性心律失常的发生机率[1]有:①自律性异常,缺血区心肌细胞代谢紊乱。②触发活动:AMI并发的心律失常约51%的病例是由触发活动引起的。③折返现象:折返现象引起的心律失常可于2 min以后发生,持续30 min以后可暂时消失。④损伤电流:健康心肌细胞与受损区细胞之间的电位差所形成损伤电流,可以引起周围普肯野细胞动作电位4相上升速度增快,诱发室性心律失常。⑤交感神经兴奋性增高,可降低室颤阈值,易发生室性心律失常。⑥室颤阈值降低:心肌缺血2 min以后,室颤阈值即可明显降低。⑦膜电位负值减少,可使0相上升速度减慢,出现束支阻滞、房室阻滞等。
