不少肺癌患友在选择免疫治疗方面犯了难——“年纪有点大,免疫治疗会不会容易引起进展啊?”“我标记物表达阴性啊,会不会不仅没效果,还引起进展呢?”“我有EGFR突变,现在耐药了想用一下PD-1抑制剂,可是有人跟我说会引起超进展。”……免疫治疗引起“超进展”让大家的抗癌信心坐上了“过山车”。那么,什么是免疫治疗超进展呢?简单来说,就是当我们使用PD-1/PD-L1抑制剂进行免疫治疗时,由于一些原因,患者的肿瘤不仅没有缩小,反而在短时间内迅速增大的现象。由于“超进展”的存在,很多患者失去了更多的治疗机会,生存曲线直线下降。彼之“蜜糖”,我之“毒药”。本来是抗肿瘤的一剂良药,如今怎么突然威胁到了生存呢?这种“超进展”的发生率究竟有多高呢?有没有什么办法可以提前预测“超进展”的发生?最近,韩国延世大学的肿瘤中心金惠润教授(P Hye Ryun Kim)试图解答这个问题,她的文章发表在最近的《肿瘤学年鉴》上。原来,免疫治疗引发“超进展”这件事,尚无定论,人们对“超进展”的定义也是五花八门。比较常用的方法是,免疫治疗的失败时间(TTF),当TTF<1~3月时,就被定义为“超进展”。然而这种方法非常的模糊。常常与进展期肿瘤的自然病程相混淆,也就意味着,不恰当的把一般进展(PD)情况归结到了免疫治疗“超进展”。为了理清这些问题,金教授对263例患者进行了系统研究,属于在亚洲研究领域排名第二的“大样本”研究。研究使用了原发病灶最长直径和体积的变化情况来综合定义“超进展”。只有在病灶增加两倍以上才认为患者出现了“超进展”。这种方法得到的生存曲线将“超进展”与“一般进展”显著区分开来:部分缓解(PR)和病情稳定(SD)的总生存期(OS)比较好,一般进展(PD)较差,超进展(HPD)最差可以看到,一般进展的患者,无进展生存期(PFS)是48天,而因免疫治疗“超进展”的患者只有19天,后者的进展风险是前者的近5倍。更严重的是,一般进展的患者近七成有机会使用别的抗癌治疗方式,超进展患者则出现了“神反转”——70%失去进一步治疗的机会!随着研究推进,金教授团队发现了许多与既往研究不太相同的地方。首先,研究没有发现年龄与超进展有任何关系。之前没有认真区分“一般进展”的报道则认为高龄(>65岁)与HPD有关。患友们或许不必因为年纪大而放弃使用免疫治疗。第二,超进展是否会引起更多器官的转移呢?答案也是否定的,研究发现,一般进展患者的器官转移数与超进展差不多。患友们似乎不该为“会不会引起更多器官转移”而忧心。第三,是不是有EGFR突变的患者更容易出现超进展呢?金教授的研究与之前的发现相反,致癌EGFR突变与超进展无关。或许因为使用过靶向药而担忧超进展也是没必要的。第四,治疗前肿瘤比较大,会不会引起超进展?研究发现,初始的肿瘤负荷与超进展也没有显著关系。而且,超进展与TMB(肿瘤突变负荷)这一用于免疫治疗预测的指标也没有相关性。如果你初始治疗时存在TMB升高,你应该庆幸免疫治疗的效果会更高,而不必担心发生超进展。当我们知道了“超进展”与“一般进展”的这种区别后,研究人员就可以像侦探一样去认真追踪,这个“超进展”究竟有什么不同之处,一旦我们发现了“超进展”的“小尾巴”,就可以不费吹灰之力,提前“揪”住它!“超进展”独有的两项生物学特征被研究团队揪了出来。通过分析超进展期患者的外周血CD8+ T淋巴细胞,人们很快发现:超进展患者的效应记忆T细胞的水平比较低,这种细胞的分子亚型是 CCR7- CD45RA-;超进展患者的衰竭型T细胞的水平比较高,这种细胞的分子亚型是TIGIT+。抓住了这两个“小辫子”,就可以成功预测超进展相关的生存威胁了。分别对这两类预测因子进行的生存分析研究特别能说明问题(以下两图)总结一下,如果你的外周血中效应记忆T细胞(CCR7- CD45RA-)表达较低,同时衰竭型T细胞(TIGIT+)高表达,那么你在使用PD-(L)1抑制剂时要特别的慎重,因为有较大几率会引“火”上身。这个“火”就是令人讨厌的“超进展”。面对风头日盛的“免疫治疗”方法,近年来发现其超进展的比例可达2%~4%。金教授他们报道的是9%。超进展并不少见,临床治疗要防范。在肿瘤精准医疗时代,或许任何靠单一治疗“神药”康复的想法都将成为一种“理想主义”。治疗前的评估、治疗过程中的监测都是必不可少的哦。
